Required Enrollment Documents
  • Certified, state-issued birth certificate (hospital footprint certificates cannot be accepted)
  • Immunization record or approved exemption documentation
  • Proof of residency within the Cushing Public Schools district

These documents are the responsibility of the enrolling family and must be uploaded with the enrollment application. Applications cannot be processed or approved until all required documents are submitted.

Although families may have provided a birth certificate to another school, birth certificates are not typically transferred between school districts during records requests. Cushing Public Schools requires the enrolling family to provide the birth certificate directly as part of the enrollment process. Enrollment applications submitted without the required documents will be unsubmitted and returned to the submitter. The application must be corrected and resubmitted with all required documents before it can be processed.

If You Do Not Have Required Documents

If you do not currently have the required birth certificate or immunization documentation, you will need to obtain a copy from the appropriate agency.

Birth certificates are typically obtained from the state vital records office where the child was born. Immunization records may be available through your healthcare provider or the Oklahoma State Department of Health immunization system.

If you must order a birth certificate, you may upload confirmation that the order has been submitted when completing enrollment. Once the document is received, email a copy to enrollment@cushingtigers.com or provide it directly to your student's school.

Rare circumstances may exist where a birth certificate or immunization record is not immediately available. If this applies to your situation, please contact the Enrollment Office for assistance.

Proof of Residency

Proof of residency must be dated within the last 30 days and must show the customer name and service address. The remittance or mailing portion of a bill cannot be accepted.

  • Gas, water, electric, internet, or cable bill
  • Gas providers: Oklahoma Natural Gas or CenterPoint Energy
  • City of Cushing utilities: water, electric, sewer, trash
  • Outside city limits utilities may be rural water districts, CREC electric, or other municipal providers
  • Internet providers must be wired services such as Optimum, Bluepeak, Centranet, AT&T, AtLink, or ProValue.net
  • Wireless or satellite services such as cellular internet, Dish Network, DirecTV, or HughesNet cannot be accepted
  • Lease agreement showing address, lease terms (dates), and signatures
  • Builder or home purchase contract showing the property address and a closing date within 90 days.
This document may be used temporarily to begin enrollment; however, after closing is completed,
a utility bill showing the customer name and service address must be provided within 60 days
to verify residency at the address.

If you move to a new address within the district, updated proof of residency will be required for the new address.

Enrollment Attendance Requirement

Students must attend school within the first three (3) days of their scheduled enrollment start date.

If a student enrolls but does not attend within the first three days, the enrollment record will be deleted and a new enrollment application will be required. CPS staff will attempt to contact the family during this time. If we are unable to make contact or receive a documented reason for the absence, the enrollment will be considered abandoned.

Student Transfers

Students who live outside the Cushing Public Schools district must receive transfer approval before submitting their enrollment application.
