Student Records Requests

This page explains how to request archived student records, official transcripts, district-to-district academic records transfers, and student information verification documents for Cushing Public Schools. Please review the options below and use the request path that best matches your need.

Archived Records, Official CHS Transcripts, and Third-Party Verifications
Archived student records, official transcript requests for Cushing High School students, and third-party or employment-related verification requests must be submitted through Parchment.
Archived student records include records for former students or records that are no longer maintained as active campus files. These requests may include official transcripts, graduation verification, and other archived student records available through Parchment.
Cushing Public Schools does not keep copies of diplomas. Diplomas are issued to graduates, and replacement diploma copies are not available through the district.
These requests are not processed by telephone, email, or walk-in request.
Requests from Other School Districts
Official student records requests from other school districts must be submitted formally by the requesting district and are processed in accordance with applicable state and federal law and district policy.
Cushing Public Schools participates in the Infinite Campus National Records Exchange (NRE). When available, requesting districts should use the appropriate Infinite Campus records-transfer process for faster and more secure exchange of official student records.
If the requesting district does not use Infinite Campus, academic records requests should be directed to the appropriate school contact below based on the student’s most recent grade level at Cushing Public Schools.
The District Administration Office cannot complete campus academic records transfers.
Pre-K through Grade 4
Records for students who last attended Pre-K through Grade 4 should be requested through the Cushing Elementary Center office.

Records for students who last attended Grades 5 through 8 should be requested through the Cushing Middle School Counseling Office.

Information to Include with a District Records Request

To help prevent delays, requests from other school districts should include enough information to identify the student, confirm the requesting district, and describe the records being requested.

  • Student full legal name
  • Date of birth
  • Last grade level attended and the last CPS campus attended
  • Name of the requesting school district and school site
  • Name, title, phone number, and email address of the requesting staff member
  • Clear description of the records needed
  • Secure return instructions or delivery method for the requested records
Processing and Timelines
Requests may be submitted year-round; however, processing times may be extended during school closures, holiday breaks, and summer months when school offices are closed or operating with limited staff.
During the summer, most campuses have minimal on-site staff until employees return in early August.

When records are requested by another school where a student seeks or intends to enroll, records may be disclosed as permitted by FERPA, and Oklahoma law requires the sending district to forward the records within three business days of receiving the request.
District Office Student Information Verification Documents

The District Administration Office cannot complete academic records transfers for campus student files. Requests for district-to-district academic records transfers should be directed to the appropriate campus contact listed above, and official transcripts or third-party verification requests should be submitted through Parchment when applicable.

The District Administration Office may provide student information verification documents upon request for current or previously enrolled students during regular business hours on open business days. These requests are typically made by guardians or other authorized requesters.

Depending on the request and available records, verification documents may include enrollment status, current physical address, current mailing address, listed guardians, relationships, birth date, projected graduation date, household information, and household membership.

These district office verification documents are not the same as official enrollment verifications used for employment purposes or requests submitted through third-party verification organizations.
